Colonoscopy: types, indications, technique and answers to frequently asked questions

Colonoscopy is an endoscopic examination of the colon.

To visualize the colon, the examination is carried out starting at the anus, using a flexible medical device (colonoscope) with a camera on the end, about the thickness of a human finger. It is used to screen for colorectal cancer and to study the causes of warning signs (blood in the stool).

Colorectal cancer screening may be indicated even in the absence of symptoms. The starting age, choice of test, and intervals depend on the adopted program, family history, and individual risk factors. Colonoscopy is one option; stool tests and other methods are also used. The preventive examination plan should be discussed with a doctor.

Colonoscopy cost

The cost of a colonoscopy depends on the clinic, type of anesthesia, need for biopsy or polyp removal, and included consumables. The previously mentioned price range is not a current offer. When making an appointment, clarify the full composition of the service, the cost of anesthetic support, and any possible additional tests; a low initial price by itself does not describe the entire scope of care.

Diagnostic and therapeutic manipulations necessary during the procedure may also not be taken into account. For example, we are talking about taking samples from the mucous membrane with further histological studies, removal of small polyps, and the like. The total cost of this diagnostic method may vary for each patient.

Cost of colonoscopy under anesthesia

The cost of a colonoscopy with sedation or general anesthesia will be clarified when making an appointment. Pain management may improve comfort but requires assessment and follow-up after the examination. The time of discharge is determined by the medical team. After sedation, a companion is usually required; driving and other restrictions are discussed in advance.

Virtual colonoscopy price

The current cost of CT colonography is clarified at the institution where the study is performed. They separately find out whether preparation, description of results and additional services are included. The method does not allow performing a biopsy or removing a polyp at the same time.

Capsule colonoscopy price

The cost of capsule endoscopy depends on the type of capsule, equipment and recording analysis. Small intestine examination and colon capsule examination have different indications. The previous fixed amount is not the current price list; The need for the method and the full calculation are clarified by a specialist.

Types of colonoscopy

Colonoscopy is an endoscopic examination of the large and rectal intestines. It allows inspection of the mucosa, tissue sampling, removal of some polyps, and in certain types of bleeding, therapeutic interventions. No method guarantees the detection of all changes: the result depends on preparation, the thoroughness of the examination, and the nature of the lesion.

Other methods of intestinal examination include capsule endoscopy, CT colonography, and sigmoidoscopy. They differ in the area of examination and capabilities and are not interchangeable types of the same procedure. More on the topic: sigmoidoscopy.

  1. Capsule colonoscopy: the patient swallows a special camera that transmits images of the mucosa. The quality depends on preparation and the complete passage of the capsule. The method does not allow taking a biopsy or removing a polyp; if findings are present, a conventional colonoscopy may be required. The decision to use it is made based on indications, without generally considering all results as unsatisfactory.
  2. Virtual colonoscopy, or CT colonography, creates images of the colon using computed tomography. Usually, bowel cleansing is required before the study; the procedure for taking contrast material is determined by the institution. Gas is introduced into the colon to expand the walls. The method usually does not require sedation, but it involves radiation exposure and has its own limitations.

However, it has disadvantages such as the ability to miss small details, exposure to radiation even at low speeds, and the need for a normal colonoscopy to intervene in polyps or other negative situations.

  • Sigmoidoscopy examines the rectum and the lower part of the colon. Preparation is necessary: the schedule, including possible use of an enema or laxative, is communicated in advance by the medical team. The examination may be prescribed for rectal bleeding and other symptoms. The patient’s age or the presence of haemorrhoids alone do not determine whether an inspection of only the lower part of the intestine is sufficient.

Colonoscopy in a dream

Sedation or general anesthesia may reduce discomfort during colonoscopy. The depth of anesthesia is selected individually; the examination is possible without sedation. After the procedure, bloating and spasms sometimes persist. The ability to drink, eat, and walk is determined by the medical team based on the manipulations performed. Severe or increasing pain, fever, or significant bleeding requires urgent evaluation rather than waiting for normal recovery.

Virtual colonoscopy (CT colonoscopy)

Virtual colonoscopy, or CT colonography, creates two-dimensional and three-dimensional images of the large intestine. This is a radiological examination with different capabilities and limitations compared to endoscopy: it does not allow simultaneous biopsy or removal of a detected lesion.

Disadvantages:

CT colonography can detect polyps but does not allow their removal. Significant findings usually require endoscopic colonoscopy. Sensitivity depends on the size, shape, and location of the lesion, as well as the quality of preparation; small or flat lesions may be missed by different methods.

Advantages:

  1. Preparation for CT colonography is also necessary; its composition and tolerability depend on the chosen protocol.
  2. In practice, anesthesia is not required.
  3. After the examination, recovery is faster.
  4. Not only the internal, but also the external surface of the intestine is visible.

Capsule colonoscopy

Capsule endoscopy uses a swallowable camera and is primarily used to examine specific sections of the digestive tract based on indications. A capsule for the small intestine and an examination of the colon are different tasks; the method does not automatically replace gastroscopy or colonoscopy. During such an examination, it is not possible to take a biopsy or remove a polyp. Although insertion of the endoscope through the anus is usually not required, there may be limitations, including the risk of capsule retention in the case of intestinal narrowing. The choice of method, preparation, and the need for subsequent examination are determined by the doctor.

This method is also used in the investigation of diseases with unexplained iron deficiency anemia, prolonged diarrhea, abdominal pain and weight loss, as well as in the diagnosis of polyps or tumors of the small intestine.

It should not be used in patients with known or suspected gastrointestinal obstruction or swallowing problems.

If you have a pacemaker or another implanted device, inform your doctor in advance. Compatibility with a specific capsule endoscopy system and monitoring conditions are clarified individually.

Colonoscopy with biopsy

If a doctor detects a suspicious area of the mucosa, an instrument can be passed through the working channel of the colonoscope to take a small tissue sample specifically from that area. The sample is sent for histological examination. The result helps to clarify the diagnosis and determine the need for further examinations or treatment.

Pain relief during colonoscopy

When there is pain in the anal area, the doctor first assesses its cause and chooses a safe method of examination and pain management. A local remedy may sometimes be used additionally, but it does not replace an individual sedation or anesthesia plan.

A colonoscopy can be performed with or without sedation depending on the clinical situation and the agreed-upon plan. During medicated sleep, the condition is monitored by the medical team; sedative drugs have contraindications and possible side effects. After the procedure, bloating or discomfort may persist. The presence of pain relief does not cancel the rules for preparation, accompaniment home, and driving restrictions after sedation.

Sedation and general anesthesia differ in the depth of consciousness suppression, response to stimuli, and required monitoring:

With moderate sedation, the patient may remain responsive to verbal communication; with deep sedation, consciousness is significantly depressed. General anesthesia implies the absence of consciousness and may require respiratory support. The expression “asleep” does not specify a particular technique. The approach is chosen based on the patient’s condition, the extent of the procedure, and the anesthesiologist’s assessment; the amount of drugs alone does not explain the differences between these states.

Indications

Colonoscopy is used for diagnosis, certain therapeutic procedures, colorectal cancer screening, and post-polyp removal follow-up. The starting age and repeat intervals are determined individually; there is no universal scheme of “every five years for everyone over fifty”.

Colonoscopy is often used in the following cases:

  • colorectal cancer screening taking into account age, the accepted program, and individual risk;
  • the appearance of blood during bowel movements;
  • detection of hidden blood in stool;
  • colon cancer or significant polyps in first-degree relatives;
  • presence of suspicious conditions detected during X-ray examination;
  • abdominal pain of unknown cause and/or excessive weight loss of unknown cause;
  • chronic diarrhea or constipation;
  • assessment of specific colorectal diseases, including situations after diverticulitis, within the timeframes and according to the indications determined by the doctor;
  • determining the causes of iron deficiency anemia;
  • follow-up after treatment for bowel cancer;
  • chronic inflammatory bowel diseases;
  • carrying out a biopsy after a virtual colonoscopy;
  • removal of foreign bodies.

Symptoms that indicate a colonoscopy

Issues such as changes in bowel habits and bleeding indicate possible problems in the colon.

Alarming symptoms:

  1. Rectal bleeding.
  2. Unexplained abdominal pain.
  3. Unexplained weight loss.
  4. Iron deficiency anemia.
  5. Bloody, mucous stool.
  6. Decreased stool diameter, defecation like goat droppings.
  7. Inability to defecate when the urge to defecate occurs when visiting the toilet.
  8. Bleeding that cannot be reliably explained by an established diagnosis; the presence of haemorrhoids does not exclude other causes.
  9. Unexplained diarrhea and changes in bowel habits.

With a family history of colon cancer or significant polyps, the doctor clarifies who and at what age they were detected and develops an individual screening plan.

Contraindications for colonoscopy

Prior to a colonoscopy, informed consent is required. Below are situations in which a scheduled examination may be postponed, its scope changed, or a different method selected after risk assessment:

  • Patients whose intestines are not properly cleansed.
  • Patients at high risk of intestinal perforation during colonoscopy.
  • Patients with severe intestinal inflammation.
  • Patients with inflammation of the peritoneum.
  • Patients with unstable cardiovascular conditions requiring urgent assessment and treatment.

Chronic illness, pregnancy, blood clotting disorders, or use of anticoagulants require individual assessment. This does not mean the same restriction applies to all patients. The doctor weighs the urgency of the examination, the risk of complications, and alternatives; medications prescribed should not be discontinued independently.

Preparation for colonoscopy

For a quality examination, the bowel must be cleaned. The medical team provides a written schedule for diet, drinking, and medication intake. The restriction of dietary fibers and the timing of stopping food depend on the chosen protocol; preparation cannot be replaced by arbitrary fasting.

A few days before the test, it may be recommended to temporarily limit foods with a lot of dietary fiber, seeds and nuts. The exact deadline and list of products follow the instructions of the institution; banning all grains and pasta for a week is not a universal rule.

On the eve of the study, follow the prescribed regimen: this may include clear liquids or low-fiber meals at certain times.

Red and purple drinks are often excluded so that they do not complicate the assessment of mucous membranes. The time for taking the cleansing solution and regular medications will be agreed upon in advance. Particular attention is required if you have diabetes, heart or kidney disease, or if you are taking medications that affect blood clotting.

On the day of the test, follow the indicated time limits for stopping food and clear liquids. They also depend on the sedation plan. Do not take additional medications or change your preparation yourself; Discuss questions about smoking and regular medications with your medical team in advance.

The basis of preparation is usually a prescribed colon cleansing solution, often taken in two stages. An enema alone does not prepare the entire colon; additional measures are applied only according to a specific doctor’s plan.

On the day of the colonoscopy, it is recommended to wear loose and comfortable clothing so that the patient is comfortable during the postoperative period.

Colonoscopy technique

Blood pressure and pulse rate are recorded by a nurse in the department where the procedure will be performed. During the procedure, the nurse inserts an IV to administer sedatives and painkillers.

To perform the colonoscopy procedure, the patient lies on the couch in the left lateral position, with the feet pulled up to the stomach.

The doctor first examines the anal area. Using gloves, he examines the last part of the colon and the anus with his finger. He then begins to examine the colon by advancing the colonoscopy device through the anus. To conduct this test, he must introduce air into the intestines.

During the examination or after it, bloating and cramping are possible. Usually they gradually decrease, but it is impossible to promise the disappearance of all complaints immediately. Severe or increasing pain, fever, severe weakness or significant bleeding requires urgent contact with a doctor.

The examination continues until the large intestine moves into the small intestine. The entire colon is examined in this way.

The procedure can be performed under mild sedation, with little pain, usually in less than an hour. After a colonoscopy, the feeling of tension and swelling decreases as the gas is removed.

If any pathology is detected, a biopsy can be performed using a special device on the colonoscope. If the polyp is visible, it can be removed using the wire loop on the colonoscopy device.
